Abstract
Ein Wellness-Analysator kommuniziert mit Sensoren, die physiologische Echtzeitdaten von einem Patienten generieren. Der Wellness-Analysator kommuniziert außerdem mit Datenbanken, die Nicht-Echtzeitinformationen bereitstellen, die für eine medizinisch bedingte Einschätzung des Patienten relevant sind. In einem diagnostischen Modus gibt eine Monitorschicht die Sensordaten ein, und Zubehörschichten geben die Datenbankinformationen ein. Zubehörschicht-Logikblöcke verarbeiten die Datenbankinformationen, um ergänzende Informationen zum Monitor auszugeben. Monitor-Logikblöcke verarbeiten die Sensordaten und die ergänzenden Informationen, um eine Wellness-Ausgabe zu generieren. Description
- Hintergrund der Erfindung
- Für die Einschätzung eines oder mehrerer physiologischer Systeme gibt es verschiedene Überwachungstechnologien. Zum Beispiel ist die Pulsoxymetrie ein allgemein akzeptiertes nichtinvasives Verfahren zur Messung der Blutsauerstoffsättigung und der Pulsfrequenz, die signifikante Indikatoren des Zustands des Kreislaufsystems sind. Pulsoxymeter, die durch bewegungsinduziertes Geräusch ausgelesen werden können, werden zumindest in den
US-Patenten Nr. 6770028 ,6658276 ,6650917 ,6157850 ,6002952 ,5769785 und5758644 offenbart, rauscharme Pulsoxymetrie-Sensoren werden in denUS-Patenten Nr. 6088607 und5782757 offenbart, alle übertragen an die Masimo Corporation, Irvine, California (”Masimo”) und hier durch Verweis einbezogen. - Physiologische Monitoren und entsprechende optische Sensoren für mehrere Wellenlängen werden zumindest in der US-Patentanmeldung Nr. 11/367013, eingereicht am 1. März 2006 unter dem Titel Multiple Wavelength Sensor Emitters (Sensor-Emitter mit mehreren Wellenlängen), und der US-Patentanmeldung Nr. 11/366208, eingereicht am 1. März 2006 unter dem Titel Noninvasive Multi-Parameter Patient Monitor (Nichtinvasives Patientenüberwachungsgerät für mehrere Parameter), beschrieben, beide übertragen an Masimo Laboratories, Irvine, CA (Masimo Labs) und beide hierin durch Verweis einbezogen.
- Ferner sind physiologische Überwachungssysteme, die rauscharme optische Sensoren und Pulsoxymetrie-Monitoren enthalten, beispielsweise einer der LNOP®-Klebe- oder Mehrwegsensoren, SofTouchTM-Sensoren, Hi-Fi TraumaTM- oder BlueTM-Sensoren und eines der Radical®-, SatShareTM-, Rad-9TM-, Rad-5TM-, Rad-5vTM- oder PPO+TM-Masimo SET®-Pulsoxymeter, alle von Masimo beziehbar. Physiologische Überwachungssysteme, die Sensoren für mehrere Wellenlängen und entsprechende nichtinvasive Blutparameter-Monitoren enthalten, wie zum Beispiel RainbowTM-Klebe- und Mehrwegsensoren und RAD-57TM- und Radical-7TM-Monitoren zur Messung von SpO2, Pulsfrequenz (PR), Perfusionsindex (PI), Pleth (= plethysmographischem) Variabilitätsindex (PVI), Signalgüte, HbCO, HbMet und Hbt und anderen Parametern, sind gleichfalls von Masimo beziehbar.
- Verschiedene andere Überwachungstechnologien können den Zustand anderer physiologischer Systeme einschätzen. Die US-Patentanmeldung Nr. 2008/0108884 A1, eingereicht am 24.09.2007, mit dem Titel Modular Patient Monitor (Modulares Patientenüberwachungsgerät), beschreibt die Überwachung von Blutbestandteil-Parametern und der Atemfrequenz sowie von Blutdruck, Blutzucker, EKG, CO2 und EEG, um einige zu nennen, und ist gleichfalls an Masimo übertragen und hierin durch Verweis einbezogen.
- Zusammenfassung der Erfindung
- Die physiologische Überwachung wird mit jeder Methodik gefördert, welche die vielen sonst ungleichartigen Technologien und Messparameter integriert. Ferner kann die physiologische Überwachung verbessert werden, indem andere Patientendaten berücksichtigt werden, zu denen die Krankengeschichte, Medikationen, Labordaten, diagnostische Testergebnisse und andere Daten außerhalb der Echtzeit-Patientenüberwachung gehören. Außerdem kann auf umfangreiche medizinische Forschung und riesige wissenschaftliche Datenbanken zugegriffen werden, um die Ergebnisse der Echtzeit-Patientenüberwachung weiter zu ergänzen und zu integrieren.
- Ein Wellness-Analysesystem integriert vorteilhaft Echtzeit-Sensordaten, die den Zustand eines oder aller Kreislauf-, Atmungs-, neurologischen, Gastrointestinal-, Harn-, Immun-, Muskel-Skelett-, endokrinen und reproduktiven Systeme eines Körpers betreffen, um eine Ausgabe zur Wellness bzw. zum Wohlbefinden zu generieren. Außerdem speichert ein Wellness-Analysesystem Registrierkurven von gemessenen Parametern während der Echtzeit-Patientenüberwachung, um die Reaktion eines Patienten im zeitlichen Verlauf zu charakterisieren, wodurch ein ”virtueller Patient” geschaffen wird, der mit simulierten Daten getestet werden kann, wodurch eine prognostische Wellness-Ausgabe entsteht.
- Ein Aspekt eines Wellness-Analysators sind Sensoren, die physiologische Echtzeitdaten von Patientenorten generieren. Ferner liefern Datenbanken Nicht-Echtzeitinformationen, die für eine medizinisch bedingte Einschätzung des Patienten von Bedeutung sind. Ein Wellness-Monitor in einem diagnostischen Modus gibt die Sensordaten ein. Zubehörschichten geben die Datenbankinformationen ein. Logische Blöcke von Zubehörschichten verarbeiten die Datenbankinformationen, um ergänzende Informationen an den Wellness-Monitor auszugeben. Logische Blöcke des Wellness-Monitors verarbeiten die Sensordaten und ergänzenden Informationen, um eine Wellness-Ausgabe zu generieren.
- Ein weiterer Aspekt eines Wellness-Analysators ist die Eingabe physiologischer Sensordaten in eine Monitorschicht. Physiologische Parameter werden auf der Basis von Sensordaten abgeleitet. Physiologische Systemzustände werden zumindest teilweise auf der Basis der Parameter generiert. Eine Wellness-Ausgabe basiert auf diesen physiologischen Systemzuständen.
- Ein weiterer Aspekt eines Wellness-Analysators ist eine Monitorschicht-Einrichtung zum Generieren eines Wellnessindexes, der auf Echtzeit-Sensordaten anspricht. Eine Zubehörschicht-Einrichtung ist an die Monitorschicht-Einrichtung gekoppelt, um den Wellnessindex der Monitorschicht-Einrichtung entsprechend Nicht-Echtzeitdaten zu ergänzen.

1 veranschaulicht ein Wellness-Analysesystem100 mit einem Wellness-Analysator110 in Kommunikation mit verschiedenen Formen von Patientendaten. Insbesondere liefern Sensoren112 Echtzeit-Patientendaten, die auf ein oder mehrere Systeme vom Kreislauf-, Atmungs-, neurologischen, Gastrointestinal-, Harn-, Immun-, Muskel-Skelett-, endokrinen und reproduktiven System eines Patienten ansprechen. Ferner liefern verschiedene lokale, regionale, nationale oder weltweite Datenbanken patientenbezogene Nicht-Echtzeitdaten. Lokale Datenbanken schließen Krankenhausaufzeichnungen120 ein, die über ein lokales Netzwerk (LAN)130 mit dem Wellness-Analysator110 kommunizieren. Krankenhausaufzeichnungen120 können die Krankengeschichte122 , neuere Labordaten und Tests124 und verordnete Medikationen und Therapien128 einschließen, um einige zu nennen. Nichtlokale Datenbanken140 enthalten patientenspezifische Daten142 und nichtspezifische Daten144 , die über ein Fernnetzwerk (WAN)150 zum Wellness-Analysator110 übertragen werden. 110 einen Patienten, indem er die Wellness-Ausgabe116 ermittelt. Auf diese Weise erzeugt der Wellness-Analysator110 ein internes Patientenmodell oder einen ”virtuellen Patienten”. Der virtuelle Patient kann dann in einem Simulationsmodus getestet werden, mit statistischen Eingabedaten anstelle von, oder zusätzlich zu, Sensordaten112 , um eine prognostische Wellness-Ausgabe118 zu ermitteln. Günstigerweise lässt die prognostische Wellness118 erkennen, wie ein Patient wahrscheinlich auf verschiedene physikalische, medizinische und Umweltbedingungen reagiert, um physiologische Stärken und Schwächen aufzudecken. Zum Beispiel kann prognostische Wellness118 ein Risiko einer unmittelbaren Verschlechterung des Gesundheitszustands enthüllen, die beispielsweise auf septischen Schock, Infektion, Embolie, Lungenentzündung und pharmazeutische Nebenwirkungen zurückzuführen ist, um einige zu nennen. Die Monitorschicht310 weist Sensoreingänge301 , Logikblöcke320 und einen Wellnessindikator-Ausgang309 auf. In dieser besonderen Ausführungsform sind die Logikblöcke320 in drei Ebenen organisiert, zu denen die Parameterebene322 , die Systemebene324 und die diagnostische Ebene328 gehören. Die Parameter(”P”)-Logikblöcke322 verarbeiten Sensordaten, um Parameter312 abzuleiten, die numerische Variable und vielleicht Wellenformen sind, die auf den Zustand verschiedener physiologischer Subsysteme schließen lassen. Zum Beispiel kann ein an einer fleischigen Gewebestelle angebrachter optischer Sensor numerische Parameter wie z. B. Sauerstoffsättigung, Pulsfrequenz und Gesamthämoglobin sowie eine plethysmographische Wellenform generieren, die mit der arteriellen und/oder venösen Durchblutung an einer bestimmten Körperstelle zusammenhängen. Diese Information liefert ihrerseits bestimmte Informationen zum Kreislaufsystem des Körpers. Obwohl ausgewählte Parameter auf dem Monitor angezeigt werden können, sind auf dieser Ebene die meisten abgeleiteten Informationen maschinenintern. - Die System(”S”)-Logikblöcke
324 reagieren, sofort und im zeitlichen Ablauf, auf Parameterwerte, Anstiege, Trends, Variabilität, Muster und Wellenform-Morphologie, um Systemindikatoren314 bereitzustellen, die einen Aspekt des physiologischen Zustands eines oder mehrerer der oben aufgeführten biologischen Systeme beschreiben, einschließlich der Kreislauf-, Atmungs- und neurologischen Systeme, um einige zu nennen. Zum Beispiel können physiologische Systemindikatoren von einer spezifizierten Anzahl von Messereignissen pro Zeiteinheit bis zu einem allgemeinen Gesundheitsmaß des physiologischen Systems variieren. Diese physiologischen Systemindikatoren sind entfernt analog zu blutchemischen Ergebnissen aus einem Labor, d. h. ein Indikator kann eine gemessene Zahl, wie z. 328 generieren eine Wellness-Ausgabe309 , d. h. eine oder mehrere Entscheidungen bezüglich der Gesundheit einer Person, die als endgültige Diagnose zur Beurteilung durch einen Pfleger vorgesehen sind. Eine Wellness-Ausgabe309 kann von einer relativ leichten Diagnose, wie z. B. ”gelegentliche Vorhofarrhythmien” über signifikante Gesundheitsprobleme, wie z. B. ”Apnoe” bis zu lebensbedrohlichen Problemen reichen, wie z. B. ”kongestiver Herzinsuffizienz”. Weitere Beispiele werden weiter unten in Bezug auf4 beschrieben. In anderen Ausführungsformen können verschiedene Typen, Anzahlen und Organisationen von Logikblöcken in der Monitorschicht310 genutzt werden. - Wie außerdem in
3 dargestellt, weisen andere Wellness-Analysatoren330 ,350 gleichfalls Logikblöcke auf, die in die Monitorschicht310 und/oder andere Schichten einspeisen. Zum Beispiel kann die Labordatenschicht330 Systemebenen-Logikblöcke aufweisen, die ermitteln, wo ein Messwert, wie z. B. ein bestimmter Blutfaktor, bezüglich eines normalen Wertebereichs liegt. Das Ergebnis wird dann in einen diagnostischen Block328 der Monitorschicht310 eingegeben, der in Kombination mit von Sensoren abgeleiteten Messungen eine bestimmte Diagnose liefert. 4 zeigt eine typische Ausführungsform eines Wellness-Monitors400 mit Sensoreingängen401 , Logikblöcken403 und einer Wellnessindikator-Ausgabe409 . Die Logikblöcke403 weisen Parameterblöcke404 , Systemblöcke406 und diagnostische Blöcke408 auf. Zum Beispiel generiert auf der Parameterebene404 ein optischer Sensor412 ein Absorptionsplethysmogramm, aus dem ein erster Parameterblock420 die Blutsauerstoffsättigung422 und die Pulsfrequenz424 generiert. Zum Beispiel kann der erste Systemblock45 zu einem bestimmten Zeitpunkt anzeigen, dass die Sauerstoffsättigung422 in Verbindung mit einer relativ stabilen Pulsfrequenz abfällt. Ein diagnostischer Logikblock480 reagiert auf die Kreislaufsystem-Ausgabe452 , indem er eine Wellnesszustandsausgabe409 generiert, die eine Meldung ”Herzschädigung” aufweist. Zum Beispiel kann der zweite Systemblock460 zu einem bestimmten Zeitpunkt anzeigen, dass die Sauerstoffsättigung422 in Verbindung mit einer steigenden Atemfrequenz abfällt. Ein diagnostischer Logikblock480 reagiert auf die Atmungssystem-Ausgabe462 , indem er eine Wellnesszustandsausgabe409 generiert, die eine Meldung ”Atemwegblockierung” aufweist. Details der Meldung warnen den Pfleger, dass eine steigende Atemfrequenz nicht zu erhöhter Sauerstoffzufuhr zu den Lungen führt. - Ferner gibt auf der Systemebene
406 ein dritter Systemblock470 die Körpertemperatur416 und den Blutdruck442 ein und generiert eine Kreislaufsystem-Ausgabe472 bezüglich der hämodynamischen Stabilität. Der dritte Systemblock470 kann außerdem eine Pulsfrequenz424 und eine Atemfrequenz432 eingeben, um eine multiple Systemausgabe472 zu generieren, die auf diese vier signifikanten Lebenszeichen anspricht. - Wie oben in Bezug auf die
1 –4 beschrieben, kann der Wellness-Analysator110 (1 ) von einem Allzweckcomputer bis zu einem Signalprozessor für Spezialzwecke oder von einer Prozessormatrix zu einem dezentralen Netz von Computern oder anderen Verarbeitungsgeräten reichen. Wie gleichfalls oben beschrieben, können der Wellness-Monitor210 (2 ) und die Zubehörschichten220 –260 (2 ) in Hardware, Software, Firmware oder einer Kombination davon implementiert werden. In einer Ausführungsform ist der Wellness-Analysator110 (1 ) ein Einzelgerät mit Einschüben, einer oder mehreren Sichtanzeigen, Tastaturen verschiedenen Standard-Kommunikationsschnittstellen, einem oder mehreren Signalprozessoren und einem Gerätemanagement-Prozessor. In einer Ausführungsform sind die verschiedenen Verarbeitungsschichten210 –260 (2 ) in Software und/oder Firmware implementiert. In einer Ausführungsform sind die verschiedenen Logikblöcke320 (3 ) Unterprogramme oder Subroutinen oder auf andere Weise identifizierbare Teile innerhalb von Software und/oder Firmware. -
5 veranschaulicht Details eines Parameter-Logikblocks500 . Wie oben in Bezug auf3 (322 ) beschrieben, weist ein Parameter-Logikblock eine Sensoreingabe501 und eine Parameterausgabe509 auf. Die Sensoreingabe501 kann eines von verschiedenen Sensorsignalen sein, wie z. B. ein Photodioden-Detektorstrom, der durch einen optischen Sensor erzeugt wird, ein piezoelektrischer Strom von einem akustischen Sensor oder eine Elektrodenspannung von einem EKG-Sensor, um nur einige zu nennen. Eine von dem Parameterblock500 getrennte Hardware-Schnittstelle510 steuert nötigenfalls den Sensor an und konditioniert, tastet ab und digitalisiert die Sensoreingabe501 zu Sensordaten512 . Ein Signalextraktionsprozess520 extrahiert ein oder mehrere physiologische Signale525 aus den Sensordaten512 . Zum Beispiel weist die Signalextraktion für einen Pulsoxymetriesensor die Demodulation der roten und IR-Signalkomponenten auf. Das (die) Sensorsignal(e)525 wird (werden) dann analysiert530 , um physiologische Parameter509 abzuleiten. Für die Pulsoxymetrie würde die Signalanalyse530 die Ableitung eines Rot/IR-Verhältnisses und die Verwendung dieses Verhältnisses zusammen mit einer Nachschlagetabelle zur Berechnung eines Sauerstoffsättigungsparameters beinhalten. -
6 veranschaulicht Details eines Systemlogikblocks600 . Wie oben in Bezug auf3 (324 ) beschrieben, weist ein Systemlogikblock Parametereingaben601 sowie, vielleicht, Sensordateneingaben auf. Der Systemlogikblock600 weist außerdem eine Systemzustand-Ausgabe609 auf. In einer Ausführungsform hat ein Systemlogikblock600 einen Eingabewähler610 , einen Merkmalextraktor620 , einen Merkmalspeicher630 und einen Merkmalanalysator640 . Eine Steuereinheit390 (3 ) stellt Steuereingaben (nicht dargestellt) für jeden Eingabewähler610 , Merkmalextraktor620 , Merkmalspeicher630 und Merkmalanalysator640 bereit, wie weiter unten ausführlicher beschrieben. In einer Ausführungsform weist der Merkmalextraktor620 eine Ebenenfunktion622 , Trendfunktion624 , Musterfunktion625 , Statistikfunktion627 und Morphologiefunktion629 auf, um einige Beispiele zu nennen. - Wie in
6 dargestellt, ermittelt der Eingabewähler610 , welche Parameterblöcke322 (3 ) seinen jeweiligen Systemblock324 (3 ) speisen. Ferner lenkt der Eingabewähler610 die ausgewählten Parameter zu bestimmten Merkmalextraktorfunktionen. Die Ebenenfunktion622 reagiert auf Eingabeparameter603 , die über einen vorgegebenen Schwellwert ansteigen oder darunter absinken. Die Trendfunktion624 reagiert auf Eingabeparameter603 mit einer positiven oder negativen Änderungsrate über einem vorgegebenen Absolutwert während eines vorgegebenen Zeitintervalls. Die Strukturfunktion625 reagiert auf ein vorgegebenes Parameterverhalten während eines vorgegebenen Zeitintervalls, wie z. B. Schwellwert-Durchgänge pro Minute. Die Statistikfunktion627 ermittelt das Parameterverhalten über eine vorgegebene Probengröße, wie z. B. Mittelwert, Varianz und Korrelation (mit anderen Parametern), um einige zu nennen. Die Morphologiefunktion629 analysiert Sensorwellenform-Charakteristiken, wie etwa dikrotisches (doppelgipfeliges) Kerbenverhalten von Plethysmogrammen als ein Beispiel. Andere, nicht dargestellte Merkmalextraktorfunktionen können eine FFT-Funktion (FFT = schnelle Fourier-Transformation) einschließen, um Frequenzcharakteristiken zu ermitteln, wie unter anderem der Nachweis einer Modulation der Sensor-Wellenform. Bei Verwendung der oben beschrieben4 als Beispiel für einen Systemlogikblock leitet der erste Systemblock450 (4 ) sowohl den Parameter SpO2 als auch den Pulsfrequenz-Parameter (PR) zur Trendfunktion624 weiter, so dass die Systemblockausgabe452 (4 ) auf Aufwärts- oder Abwärtstrends sowohl von SpO2 als auch der Pulsfrequenz reagiert. - Wie ferner in
6 dargestellt, gibt der Merkmalanalysator640 die extrahierten Merkmale607 ein, um das Funktionieren eines physiologischen Systems anzuzeigen, wie z. B. der oben zitierten Kreislauf-, Atmungs-, neurologischen und anderen Systeme. In einer Ausführungsform ist die Zustandsausgabe609 eine Liste von Zustandscodes, die anzeigen, dass ein bestimmtes physiologisches System in Ordnung ist oder sich in verschiedenen Graden von Notsituationen befindet. In einer Ausführungsform greift der Merkmalanalysator640 auf eine Nachschlagetabelle650 zu, um die extrahierten Merkmalen entsprechenden Zustandscodes zu generieren, In einer Ausführungsform werden die Zustandscodes entsprechend der Schwere priorisiert. In einer Ausführungsform wird die Nachschlagetabelle650 von der Steuereinheit390 (3 ) entsprechend bestimmten Systemblock-Eingaben und -Funktionen hochgeladen. - Wie gleichfalls in
6 dargestellt, speichert der Merkmalspeicher Kurven von ausgewählten Eingabeparametern zusammen mit extrahierten Parametermerkmalen. Auf diese Weise baut jeder der Systemblöcke600 eine Charakterisierung des Patienten in Bezug auf das jeweils überwachte physiologische System auf. Die Summe dieser Patientencharakterisierung über alle Systemblöcke erzeugt einen ”virtuellen Patienten”, der durch einen Simulator870 (8 ) getestet werden kann. - Während der Simulation werden unabhängige Parameter
601 und möglicherweise einige Sensordaten simuliert, und abhängige Parameter werden entsprechend aus dem Merkmalspeicher630 ”abgespielt”. Der Eingabewähler610 kombiniert die simulierten Parameter601 und die Wiedergabeparameter632 als Eingaben für den Merkmalextraktor620 . Der Merkmalanalysator640 reagiert dann auf die simulierten extrahierten Merkmale607 , um je nachdem, wie der Patient gemäß seiner Krankengeschichte reagiert, einen entsprechenden Systemzustand609 zu bestimmen. Die Simulation wird weiter unten in Bezug auf8 ausführlicher beschrieben. -
7 veranschaulicht Details eines diagnostischen Logikblocks700 mit Systemzustands-Eingaben701 und einer Wellness-Ausgabe703 . Der diagnostische Block700 weist ein Expertensystem710 , eine diagnostische Wissensdatenbank720 und einen Ausgabegenerator730 auf. Das Expertensystem710 empfängt die Eingaben zum Systemzustand701 , die durch die verschiedenen Systemlogikblöcke600 (6 ) generiert werden. In einer Ausführungsform wird der Systemzustand701 in Form von Zustandscodes eingegeben, wie oben beschrieben. Das Expertensystem710 stellt die Zustandscodes zusammen und interpretiert diese Codes entsprechend der diagnostischen Wissensdatenbank720 , um eine diagnostische Ausgabe712 zu generieren. In einer Ausführungsform besteht die diagnostische Ausgabe712 aus einem oder mehreren diagnostischen Codes, die eine kompilierte diagnostische Interpretation der Systemzustandscodes sind. Der Ausgabegenerator730 bestimmt Form und Format der Wellness-Ausgabe703 gemäß den diagnostischen Codes712 . In einer Ausführungsform besteht die Wellness-Ausgabe703 aus einer oder allen von den folgenden Informationen: einem Wellnessindex732 , einer oder mehreren diagnostischen Meldungen734 und Alarmen und Sichtanzeigen738 . In einem prognostischen Wellness-Modus wird die prognostische Wissensdatenbank740 anstelle der diagnostischen Wissensdatenbank720 verwendet, wie weiter unten in Bezug auf8 beschrieben. - In einer Ausführungsform ist ein Wellnessindex
732 eine numerische, alphanumerische, Farb- oder andere Skala oder eine Skalenkombination, welche die Gesamtsumme einer diagnostischen Ausgabe712 verkörpert. Zum Beispiel kennzeichnet ein Wellnessindex10 , dass keine erheblichen Probleme oder Sachverhalte durch irgendeinen Systemblock gemeldet werden; ein Wellnessindex8 kennzeichnet ein geringfügiges Problem, das durch mindestens einen Systemblock gemeldet wird; ein Wellnessindex6 kennzeichnet ein durch mindestens einen Systemblock gemeldetes erhebliches Problem oder durch mehrere Systemblöcke gemeldete kleinere Probleme; ein Wellnessindex4 kennzeichnet durch mehrere Systemblöcke gemeldete erhebliche Probleme; und ein Wellnessindex2 kennzeichnet ein durch mindestens einen Systemblock gemeldetes großes Problem. - In einer Ausführungsform ist eine diagnostische Meldung
734 ein Text in natürlicher Sprache, der eine klinische Entscheidung als Reaktion auf Systemzustandscodes701 anzeigt. Zum Beispiel könnte eine diagnostische Meldung eine mögliche Ursache für spezifische Symptome angeben, wie z. B. ”Patient zeigt Symptome von Apnoe”. In einer Ausführungsform könnte die diagnostische Meldung734 auch ein Arzneimittel für die Symptome oder einen möglichen Behandlungsverlauf vorschlagen. - In einer Ausführungsform sind Alarme und Sichtanzeigen
738 irgendwelche akustischen oder visuellen Indikatoren, die allein oder in Kombination mit anderen Wellness-Zustandsausgaben732 ,734 funktionieren, um Details bezüglich der Gesundheit oder Krankheit des Patienten einem Pfleger mitzuteilen. Alarme schließen Töne mit einer einzigen Frequenz, Mischfrequenz oder veränderlicher Frequenz ein. Sichtanzeigen schließen Text, Balkendiagramme, Diagramme mit kartesischen, Polar- oder 3-D-Koordinaten und Piktogramme ein, um einige zu nennen. -
8 veranschaulicht einen prognostischen Wellness-Analysator800 mit einem Monitor810 , einer oder mehreren Zubehörschichten830 ,850 und einem Simulator870 . Der Monitor810 weist Parameter-Logikblöcke804 , System-Logikblöcke806 einen oder mehrere diagnostische Logikblöcke808 , eine Steuereinheit807 und eine prognostische Wellness-Ausgabe809 auf. In einer Ausführungsform nutzt der prognostische Wellness Analysator800 die gleichen Hardware/Software-Ressourcen wie der Wellness-Analysator300 (3 ). Das heißt, in einem prognostischen Wellness-Modus deaktiviert die Steuereinheit807 eine oder mehrere der Sensoreingaben801 und/oder Parameter-Logikblöcke804 und aktiviert den Simulator870 . Die Steuereinheit807 konfiguriert außerdem die System-Logikblöcke806 , um auf gespeicherte Merkmale630 (6 ) zuzugreifen, und konfiguriert den diagnostischen Block700 (7 ), um die prognostische Wissensdatenbank740 (7 ) anstelle der diagnostischen Wissensdatenbank720 (7 ) zu nutzen. So konfiguriert, testet der prognostische Wellness-Monitor810 vorteilhaft einen virtuellen Patienten, der aus einer Vorgeschichte von Patientenüberwachung und -informationen konstruiert wird, die durch die Zubehörschichten830 ,850 bereitgestellt werden, um kurzfristige Gesundheitsrisiken und vielleicht eine längerfristige Anfälligkeit für Erkrankung und andere Gesundheitsbedrohungen des Patienten einzuschätzen, wie oben beschrieben. - Wie in
8 dargestellt, synthetisiert der Simulator870 eine oder mehrere Sensoreingaben801 und/oder eine oder mehrere Parameterausgaben804 . Zum Beispiel kann der Simulator870 einen ersten Parameterblock812 generieren, der gemäß einer ersten Wahrscheinlichkeitsverteilung ausgegeben wird; einen zweiten Parameterblock814 deaktivieren und einen n-ten Parameterblock818 generieren, der gemäß einer n-ten Wahrscheinlichkeitsverteilung819 ausgegeben wird. In einem auf4 basierenden Beispiel wird die Sauerstoffsättigung422 (4 ) etwa zwischen 85 und 95% variiert, und die Pulsfrequenz424 (4 ) wird etwa zwischen 100 und 150 Schlägen pro Minute variiert. Entsprechend können Atemfrequenz432 (4 ), Temperatur416 (4 ) und Blutdruck442 (4 ) als abhängige, auf dem simulierten Blutdruck und der simulierten Atemfrequenz basierende Parameter variieren. Das heißt, die nicht simulierten Parameter variieren entsprechend den historischen (gespeicherten) Reaktionen des Patienten auf ähnliche Veränderungen von Sauerstoffsättigung und Pulsfrequenz. Auf diese Weise wird die wahrscheinliche Reaktion des Atmungssystems und der Hämodynamik eines Patienten auf normale Veränderungen der Sauerstoffzufuhr gemessen. - Wie ferner in
8 dargestellt, speisen die simulierten Parameter813 ,819 die Systemlogikblöcke806 . Im prognostischen Modus verändert die Steuereinheit807 die Wege des Systemblocks806 so, dass die Systemzustandsausgaben805 nur auf die simulierten Parameter813 ,819 und die gespeicherten, im Wellness-Modus durch den Merkmalextraktor620 (6 ) aufgezeichneten Merkmale632 (6 ) ansprechen. Daher lösen die extrahierten Merkmale von simulierten (unabhängigen) Parametern die Abfrage von abhängigen Parameter-Merkmalen aus, die auf einer überwachten Vorgeschichte basieren. Entsprechend generieren die Systemblöcke806 einen Systemzustand805 , der sowohl die simulierten Parametereingaben813 ,819 als auch die historische Reaktion des Patienten auf diese Parameter widerspiegelt. - In Fortführung des auf
4 basierenden Beispiels: da Sauerstoffsättigung und Pulsfrequenz durch den Simulator870 generiert werden, generiert der Merkmalspeicher der Systemblöcke806 im ”Wiedergabemodus” intern die entsprechenden ”vorausgesagten” Atemfrequenz-, Temperatur- und Blutdruck-Parameter. Der entsprechende Systemzustand805 wird in den diagnostischen Block808 eingegeben, der eine prognostische Wellness-Ausgabe809 generiert, die sich von der Wellness-Ausgabe703 (7 ) unterscheidet. In einer Ausführungsform zeigt anstelle einer klinischen Diagnose eines bestehenden Zustands auf der Basis der diagnostischen Wissensdatenbank720 (7 ) die prognostische Wellness die Wahrscheinlichkeit einer kurzfristigen Verschlechterung eines bestehenden Zustands entsprechend der prognostischen Wissensdatenbank740 (7 ) an, wie z. B. ein Risiko für Infektion, septischen Schock oder eine Arzneimittelnebenwirkung, um einige zu nennen. - Ein Wellness-Analysesystem ist in Verbindung mit verschiedenen Ausführungsformen ausführlich offenbart worden. Diese Ausführungsformen werden lediglich als Beispiele offenbart und sollen den Umfang der nachstehenden Patentansprüche nicht einschränken. Für den Durchschnittsfachmann werden viele Veränderungen und Modifikationen ersichtlich sein.

Claims (20)
- Wellness-Analysator, der aufweist: mehrere Sensoren, die physiologische Echtzeitdaten von mehreren Stellen an einem Patienten generieren; mehrere Datenbanken, die Nicht-Echtzeitinformationen bereitstellen, die für eine medizinisch bedingte Einschätzung eines Patienten relevant sind; einen Wellness-Monitor, der in einem diagnostischen Modus die Sensordaten eingibt und eine Wellness-Ausgabe generiert; mehrere Zubehörschichten, welche die Datenbankinformationen eingeben, die als ergänzende Informationen zum Wellness-Monitor ausgegeben werden; mehrere Zubehörschicht-Logikblöcke, welche die Datenbankinformationen verarbeiten, um die ergänzenden Informationen zu generieren; und mehrere Wellness-Monitor-Logikblöcke, welche die Sensordaten und ergänzenden Informationen verarbeiten, um die Wellness-Ausgabe zu generieren.
- Wellness-Analysator nach Anspruch 1, der ferner aufweist: mehrere Parameterblöcke, die als erste Ebene der Wellness-Monitor-Logikblöcke so organisiert sind, dass sie die Sensordaten eingeben und mehrere Parameter als Ausgaben generieren; eine Signalextraktionsfunktion der Parameterblöcke, die physiologische Signale aus den Sensordaten extrahiert; und eine Signalanalysefunktion der Parameterblöcke, welche die Parameter aus den physiologischen Signalen ableitet.
- Wellness-Analysator nach Anspruch 2, der ferner aufweist: mehrere Systemblöcke, die als zweite Ebene der Wellness-Monitor-Logikblöcke so organisiert sind, dass sie die Parameter eingeben und mehrere Systemzustandsausgaben generieren; und jede Systemzustandsausgabe den physiologischen Zustand eines biologischen Systems anzeigt, das eines der folgenden Systeme einschließt: Kreislauf-, Atmungs-, neurologisches, Gastrointestinal-, Harn-, Immun-, Muskel-Skelett-, endokrines und reproduktives System des Patienten.
- Wellness-Analysator nach Anspruch 3, der ferner mindestens einen diagnostischen Block aufweist, der als dritte Ebene der Wellness-Monitor-Logikblöcke so organisiert ist, dass er die Systemzustände eingibt und die Wellness-Ausgabe generiert.
- Wellness-Analysator nach Anspruch 5, der ferner aufweist: einen Simulator, der mindestens eine simulierte Parameterausgabe generiert; wobei die Systemblöcke und der mindestens eine diagnostische Block, wenn sich der Wellness-Monitor im Simulationsmodus befindet, auf den mindestens einen simulierten Parameter ansprechen, um eine prognostische Wellness-Ausgabe zu generieren.
- Wellness-Analysator nach Anspruch 5, wobei jeder der Systemblöcke aufweist: eine Merkmalextraktionsfunktion, die auf die Parameter anspricht, um mindestens eine(n) der Parameterebenen, Trends, Muster und Statistiken als extrahierte Merkmale zu identifizieren; einen Merkmalspeicher, der die extrahierten Merkmale im diagnostischen Modus speichert; und eine Merkmalwiedergabe, welche die extrahierten Merkmale im Simulationsmodus abruft.
- Wellness-Analysator nach Anspruch 6, wobei der mindestens eine diagnostische Block aufweist: eine diagnostische Wissensdatenbank, die diagnostische Codes speichert; eine prognostische Wissensdatenbank, die prognostische Codes speichert; und ein Expertensystem, das mit der diagnostischen Wissensdatenbank und der prognostischen Wissensdatenbank kommuniziert; wobei das Expertensystem im diagnostischen Modus auf die Systemzustandsausgaben reagiert, um auf der Basis der diagnostischen Wissensdatenbank die Wellness-Ausgabe zu generieren; und wobei das Expertensystem im Simulationsmodus auf die Systemzustandsausgaben reagiert, um auf der Basis der prognostischen Wissensdatenbank die prognostische Wellness-Ausgabe zu generieren.
- Wellness-Analyseverfahren, das aufweist: Eingabe von physiologischen Sensordaten in eine Monitorschicht; Ableiten mehrerer auf den Sensordaten basierender physiologischer Parameter; Ausgabe mehrerer physiologischer Systemzustände, die zumindest teilweise auf den Parametern basieren; und Generieren einer Wellness-Ausgabe, die auf den Systemzuständen basiert.
- Wellness-Analyseverfahren nach Anspruch 8, wobei das Generieren beinhaltet: Extrahieren mehrerer Merkmale aus den Parametern; Analysieren der Merkmale, um die Systemzustände zu bestimmen, und Ablegen der Merkmale in einem Speicher.
- Wellness-Analyseverfahren nach Anspruch 9, wobei die Extraktion beinhaltet: Weiterleiten der Parameter zu ausgewählten Merkmalextraktorfunktionen, und Durchführen der Extraktorfunktionen an den Parametern; wobei die Funktionen zumindest Ebenen-, Trend- und statistische Berechnungen einschließen.
- Wellness-Analyseverfahren nach Anspruch 10, wobei die Analyse das Auslesen mehrerer Systemzustandscodes aus einer Nachschlagetabelle entsprechend den extrahierten Merkmalen beinhaltet.
- Wellness-Analyseverfahren nach Anspruch 11, wobei das Generieren eines Wellness-Zustands beinhaltet: Bereitstellen eines Expertensystems in Kommunikation mit einer diagnostischen Wissensdatenbank; Interpretieren der Systemzustandscodes mit dem Expertensystem, um einen diagnostischen Code zu generieren; und Erzeugen mindestens einer der Informationen Wellnessindex, diagnostische Meldung und Alarm auf der Basis des diagnostischen Codes.
- Wellness-Analyseverfahren nach Anspruch 12, das ferner aufweist: Eingabe von Nicht-Echtzeitdaten in eine Zubehörschicht; Ableiten einer auf den Nicht-Echtzeitdaten basierenden Zubehörschicht-Ausgabe; und Generieren mindestens eines der zumindest teilweise auf der Zubehörschicht-Ausgabe basierenden Systemzustände.
- Wellness-Analyseverfahren nach Anspruch 13, das ferner aufweist: Simulieren unabhängiger Parameter; Wiedergabe von abhängigen Parametern gemäß den gespeicherten Merkmalen; und Bestimmen einer prognostischen Wellness-Ausgabe als Reaktion auf die simulierten und die wiedergegebenen Parameter.
- Wellness-Analysator, der aufweist: eine Monitorschicht-Einrichtung zum Generieren eines Wellnessindexes als Reaktion auf Echtzeit-Sensordaten; und eine mit der Monitorschicht-Einrichtung gekoppelte Zubehörschicht-Einrichtung zum Ergänzen des Monitorschicht-Wellnessindexes entsprechend Nicht-Echtzeitdaten.
- Wellness-Analysator nach Anspruch 15, wobei die Monitorschicht eine Parameterblock-Einrichtung zum Extrahieren eines Signals aus Sensordaten und zur Analyse des Signals aufweist, um einen physiologischen Parameter zu generieren.
- Wellness-Analysator nach Anspruch 16, wobei die Monitorschicht ferner eine Systemblock-Einrichtung zum Extrahieren eines Merkmals aus dem physiologischen Parameter und zur Analyse des Merkmals aufweist, um einen physiologischen Systemzustand zu generieren.
- Wellness-Analysator nach Anspruch 17, wobei die Monitorschicht ferner eine diagnostische Blockeinrichtung zum Kompilieren des Systemzustands aufweist, um eine diagnostische Ausgabe zu generieren.
- Wellness-Analysator nach Anspruch 18, der ferner eine Merkmalspeichereinrichtung zum Speichern von extrahierten Merkmalen in einem diagnostischen Modus und zur Wiedergabe dieser Merkmale in einem Simulationsmodus aufweist.
